Yes you will need a referral from your practitioner in order for us to provide you with relevant imaging services.

If you are experiencing difficulties uploading or you have a paper copy of your referral, fill in the above information and a friendly staff member will be in contact shortly to arrange your appointment. Alternatively you can drop into the clinic to book your appt and drop off your referral. Otherwise remember to have your referral with you on the day of your appointment.

Our online appointment request form is available 24 hours a day. If the request is sent through outside our business hours of Monday – Friday 8:00am to 5:30pm (EST), we will contact you on the next business day (with the exception of public holidays).
